Download and complete the Peptide Purity Testing Intake Form. This form provides the information needed to correctly process your submission and generate your Certificate of Analysis.
Important: The Company / Organization Name
listed on the intake form will be the only name displayed on the Certificate of Analysis (COA).

Step 2 — Prepare Your Samples
- Ensure vials are securely sealed and protected during shipping.
- Clearly label each vial.
- Confirm lot numbers match the intake form.
- If submitting multiple vials for batch conformity testing ensure they are from the same lot.
Step 3 — Ship Your Samples
Please include the following in your shipment:
- Completed Intake Form
- Properly labeled sample vials
